Dates Wages Paid – The date wages paid entries must have at least one date entered in the following format MM/DD/CCYY (ex 12/08/2004). Users can enter up to four wage paid dates.

Filing Period – The filing periods will be displayed in a drop down box. The dates will designate the quarter, which the payment is being applied. The available quarters are the most current quarter plus the prior three quarters.

City Entries – The amount for all city entries must be in dollar and cents.

Total – All Cities – The total of all city entries will be displayed as a total at the bottom of the page. 

	The Back Button will take the user to the previous page  (Main Menu)
	The Calculate Button will update the amounts entered and validate all business rules for the IT-15 form. The page will refresh and display the results.
	The  Continue Button will update the amounts entered, validate all business rules for the IT-15 form and take the user to the IT-15 verification page.
